All applicants are required to complete and submit their registration at the Academy by filling out and submitting this form upon entry. This form provides the minimum data for entry onto the rolls and will be used to establish the applicant's record at the Players Guild upon graduation

Please provide the following information:  Name (for the roster); State you claim for residency within CWOL; What National Allegiance you claim; your preferred Role to play within CWOL (Army, Navy, Political - state or national, economic (industrialist, plantation owner, blockade runner captain); the Email Address you will use to play CWOL .
